Membership Terms and Conditions

Your yearly fee will be $1500 (VIP Gold Membership) or $3000 (VIP Platinum Membership). Payment is due in full at the start of your membership year OR you can pay monthly using external financing with no interest.

We require a 90 day notice for membership cancellation. Cancellation must be requested in writing. If a client moves outside of a 30 mile radius, the cancellation fee will be waived as long as proof of move is given in writing.

The membership starts the day you sign up and renews on that day each year moving forward. If membership is canceled before 12 months, clients will be charged a cancellation fee of $300, regardless of the months that are remaining in the membership term. At the end of the 12 month membership term, the client will be automatically re-enrolled for another 12 month term unless the 90 day written notice has already been provided to Hollywood Eye Institute.

Other than 'Maui Money', discounts and membership benefits are non transferable.

Any membership fee charge that is denied or returned for incorrect card number or card information is considered outstanding and membership services and discounts will not be available until balance is paid in full. Membership fee can be paid with a valid credit card, cash or certified check. If you wish to pay monthly, external financing with Care Credit is available with no interest.

Membership payments are non-refundable.
